At Premier Vascular, your care is provided through a coordinated team approach designed to give you the quickest access to high-quality vascular expertise. New and established patients may be seen by a member of our vascular care team, which includes Physician Assistants (PAs) and Nurse Practitioners (NPs) with more than 20 years of combined experience in vascular medicine.
These advanced practice providers play a key role in delivering comprehensive vascular care. They evaluate patients, review imaging, answer questions, and help guide your treatment plan. Your care may be managed by one of these providers, with oversight and collaboration from our vascular surgeons as needed to ensure the right plan for you.
Cade Brownlee, FNP-C – Nurse Practitioner
Cade Brownlee is a board-certified Family Nurse Practitioner specializing in vascular and endovascular care. A native of Macon, Georgia, Cade has spent most of his life in the Middle Georgia area and is proud to serve the community he calls home.
Cade provides care for patients with a wide range of vascular conditions, including carotid artery disease, peripheral artery disease (PAD), venous insufficiency, and dialysis access needs. He also has experience assisting with minimally invasive endovascular procedures and is committed to helping patients improve their vascular health and quality of life through personalized, evidence-based care.
Known for his approachable and compassionate nature, Cade values building strong relationships with his patients and guiding them through every step of their treatment journey.
Outside of the office, Cade enjoys fishing, watching college football, and spending time with his family.
Wade Carr, FNP-C – Nurse Practitioner
Wade Carr is a board-certified Family Nurse Practitioner dedicated to providing compassionate, patient-centered care. He earned his Associate of Science in Nursing from Middle Georgia State University in 2019, followed by his Bachelor of Science in Nursing in 2021. He later completed his Master of Science in Nursing at Walden University in 2025.
Wade’s clinical approach emphasizes evidence-based practice, health promotion, disease prevention, and the development of individualized treatment plans tailored to each patient’s unique needs. He is committed to building strong therapeutic relationships with patients and their families, providing professional, compassionate care while helping individuals achieve their health goals and improve their quality of life.
Outside of caring for patients, Wade enjoys spending time with family and serving his community.
Sarah Crosby, PA-C – Physician Assistant
Sarah Crosby is a Physician Assistant at Premier Vascular with over a decade of experience in Vascular Surgery. Originally from Gainesville, GA, Sarah spent much of her career serving patients in Lawrenceville, GA before relocating to Middle Georgia after marrying her husband last year.
Sarah earned her Associate’s degree in Education from Young Harris College in 2008 as well as her Bachelor’s degree in Biology from Brenau University in 2011 before completing her Master’s degree in Physician Assistant Studies at Philadelphia College of Osteopathic Medicine in 2015.
Sarah’s passion for medicine began at an early age through the influence of her mother, a radiology technologist who worked in the cardiac cath lab. Watching her mother dedicate herself to patient care inspired Sarah to pursue a career in healthcare. During her surgical rotations in school, she discovered vascular surgery and immediately knew it was where she was meant to be. Since beginning her career in 2015, she has found deep fulfillment in caring for patients through every stage of their treatment journey, building lasting relationships and helping them heal and regain quality of life.
Known for her compassionate and approachable nature, Sarah takes great pride in truly knowing her patients and their stories. She values the trust they place in her and considers it one of the most rewarding parts of her profession.
Outside of medicine, Sarah is a former college softball player who still enjoys playing recreationally on weekends and coaching young athletes whenever she can. She recently welcomed her first child, a baby boy, and says becoming his mom has been the greatest joy of her life.
Melissa McPhail, FNP-BC – Nurse Practitioner
Melissa McPhail is an ANCC board-certified Family Nurse Practitioner specializing in vascular surgery. She has been a Registered Nurse since 1997, bringing nearly three decades of healthcare experience to her role. Her clinical background includes emergency medicine and critical care, where she developed extensive experience caring for patients with complex and acute medical conditions.
Melissa earned her Master of Science in Nursing with a Family Nurse Practitioner concentration from Kennesaw State University in 2009. After completing her advanced practice training, she spent several years providing care in family practice and urgent care settings before transitioning to vascular surgery in 2014.
Melissa is passionate about delivering high-quality, patient-centered care and helping individuals navigate their vascular health journey. She is committed to building meaningful relationships with her patients, providing education, and partnering with them to achieve the best possible outcomes.
Angela Polk, FNP-C – Nurse Practitioner
Angela Polk is a board-certified Family Nurse Practitioner with more than 40 years of nursing experience. She has been practicing as a Family Nurse Practitioner since 2017 and has worked in a variety of healthcare settings throughout her career. Her clinical background includes cardiology, endocrinology, gastroenterology, and pulmonology, providing her with extensive experience caring for patients with both acute and chronic medical conditions.
Angela earned her Bachelor of Science in Nursing from Samford University in Birmingham, Alabama, and later completed her Master of Science in Nursing at Troy University.
Angela is passionate about patient education and believes that informed patients are empowered to make healthier choices. She enjoys partnering with individuals and families to develop personalized care plans, improve health outcomes, and help patients achieve their wellness goals.
Outside of her professional practice, Angela is dedicated to serving her community through volunteer work with a local dog rescue organization, where she also serves as the fundraising coordinator. She takes pride in helping animals find loving homes while supporting rescue efforts through community engagement and fundraising initiatives.
